  • List of any medications you are currently taking

  • If Veteran: Veterans Affairs Canada, VAC Card

  • Extended Medical Card if Blue Cross or Greenshield for orthotic paperwork

 

Methods of payments we accept are:

Visa, MasterCard, Debit, and Cash.

Receipts will be provided.

We do not direct bill to insurance companies but will provide all required paperwork.

Cancellation Policy / No Shows:

We have a 48 hour cancellation policy, if you no show or cancel within 48 hours of your booked appointment there most likely will be a $60 charge. Appointments made are the responsibility of the patient regardless if you received a courtesy reminder or not. To reschedule simply call the office: 250-592-0224. You can leave a message if we are not in the office for cancellations. We do not accept email cancellations. 

Patient Behavior / Abuse:

Patients who are rude, aggressive, argumentative, difficult, or abusive will be terminated from my practice. My Medical Office Assistant is not here to be abused.  She is following my office policies and procedures. Please be respectful and she will be respectful as well.

Orthotic Extended Benefits:
We provide the following to support your claim (not including 2nd pairs):

  1. Biomechanical Exam

  2. Rx and Diagnosis

  3. Casting Technique - Plaster of Paris slipper cast

  4. Dispensing Date

  5. Receipt(s) - Paid In Full

  6. Precision Orthotics - Lab Invoice


Please keep in mind that most insurance plans require a dispensing date and lab invoice for reimbursement and approval. If your plan is about to expire, please also consider this when booking your appointment. We do not alter insurance paperwork due to insurance expiring etc...
